PROGRAMMING ADVISERS - APPLICATION FORM

NB:

• The number of places is limited, so registration will be made in order of receipt of the forms. The allocation of hotel will depend on the availability of places and general organisational criteria.
• Professionals attending will be charged as follows:
  • Double room with breakfast: €30 per person and night.
  • Individual room with breakfast: €55 per person and night.
  Payment is to be made when checking-in at reception in the allocated hotel.
• The festival will provide the following:
  • Subsidised accommodation.
  • Lunch everyday.
  • Tickets for shows with limited seating.
  • Free internet access points, through its Wi-Fi laptop service.
  • Entry to the Meeting Point.
• Each guest institution may be represented by one person. In the event that more people attend, they are to meet the full cost of their accommodation and their registration is to be submitted on separate forms.
• You are advised to provide a mobile phone number so that, if necessary, we can contact you immediately.
• In the event that you are unable to attend on the dates indicated, please inform the Festival office as soon as possible.
• The deadline for submitting this application is 2 May 2011.
• Once the application form for the Umore Azoka has been received, the Organization will send a confirmation e-mail; if you do not receive one, please contact us (telephone: 94 607 25 70).
